Why does my image look different in every app?

I created a CinamaDNG video (a folder of DNG images) from scratch (i.e. from generated RGB values) using code similar to https://forums.adobe.com/thread/2105577. The resulting component images

• Look correct in RawTherapee 4.2.976 (although the preview image is sometimes wrong)
• Open but look wrong (washed out and colors aren't correct) in Windows Photo Viewer
• Validate without error with dng_validate.exe

and the CinemaDNG video

• Opens but looks completely wrong in DaVinci Resolve (it's black and white)
• Fails to open in Adobe Premier Pro CC (error "Unsupported format or damaged file")

This is pretty frustrating. What's wrong with my image/video?
